  Initial Registration Form

If you are a Lay Professional trained for ministry or working in the Episcopal Church (e.g. Director of Religious Education, musician, parish administrator, teacher, diocesan staff), you need your Bishop’s certification to register\:

1) Download Bishop's Certification Form for Lay Professionals (below).
2) Have your diocesan bishop or deployment/transitions officer sign and mail it to: CDO, 815 Second Avenue, New York, NY 10017.
3) CDO will notify you that you may complete the Initial Registration Form online or download this form and mail it to CDO.
4)   When CDO receives your initial registration form, a UserName and password will be emailed to you with a link to complete the registration online.
You may want to download the On Line User’s Manual, the Ministry Skill/Experience Manual, and the Long Registration Form (as a worksheet).
5)   If you order a Registration Kit in the mail, you will receive all three documents in #4 above.

OR

1) Download the Long Registration Form. This and other forms are available at CDO Forms.
2) Download and review Ministry Skills/ Experience Manual.
3) Complete and mail the Long Registration Form to: CDO, 815 Second Avenue, New York, NY 10017.
4) CDO will then create and send you your Personal Profile. You will receive a User Name and password for accessing your profile online.
